Privacy Policy of Human Services Programs of Carroll County Inc.

Please be advised that all clients served through Human Services Programs of Carroll County Inc. (HSP) will have their service file recorded on Community ServicePoint.

What is Community ServicePoint?

    Community ServicePoint is an Internet based information storage system that is available to several other Carroll County Agencies for the purpose of securely holding information about the people they serve.

Why is HSP using Community ServicePoint?

    HSP is using this system to better understand and meet our clients’ needs and to help us plan for and adjust appropriate resources for persons requesting services as well as provide helpful information to make public policy.

Security

      All information that is electronically transferred over the internet in this system is protected through a secure connection.

      Only staff who work for HSP can see or change the information given to HSP.

      In order for this community data information system to be a benefit to those we serve in Carroll County, your name and the last four digits of your social security number could possibly be seen by another Carroll County Agency who uses this system. The partnering agency would not be able to access any other data entered at HSP or know which agency previously entered your name.

Privacy Protection

      Aside from full name and last four digits of social security number, your information is not shared with any other agency without written consent.

      You have the right not to answer any questions that are not required to receive services

      You have the right to know who has added to deleted or change your Community ServicePoint record.

Benefits to You

      Case managers can quickly identify what services can be offered to you here and track the services to maintain good customer service.

      Using Community ServicePoint, HSP can access information about other agencies and the services they may be able to provide to you.

Written Consent and Release Information

      HSP will not release personal information about you or your household to outside agencies without you first signing a written consent that specifies what information you wish to be given out and to whom.

      HSP must adhere to the below federal and state law.

Federal / State Confidentiality Regulations:

1.) The Participating Agency will abide specifically by the Federal confidentiality rules as contained in 42 CFR part 2 regarding disclosure of alcohol and/or drug abuse records. In general terms, the Federal rules prohibit the disclosure of alcohol and drug abuse records unless disclosure is expressly permitted by written consent of the person to whom it pertains or as otherwise permitted by 42 CFR part 2. A general authorization for release of medical or other information is not sufficient for this purpose. The participation Agency understands that the Federal rules restrict any use of the information to criminally investigate or prosecute any alcohol or drug abuse patients. 2) The Participating Agency will abide specifically by State of Maryland general laws. 3) Solicitation of Unnecessary Information: The Participating Agency will not solicit or input information from clients unless it is essential to provide services or conduct evaluation or research.
